January 26, 1980 letter from J. G. Keppler to C. Reed transmitting IE Inspection Report Nos. 50-454/79-18 and 50-455/79-18
Dear Mr. Keppler:
Reference (a) transmitted an inspe-ion report regarding an inspection conducted on August 28, Septembe. 17-28, October 11-12, and November 5, 1979 by Messrs. J. E. Foster and P. A. Barrett of your office of activities at Byron Station.
That report identified two apparent items of noncompliance with NRC requirements.
Attachment A to this letter contains Commonwealth Edison Company's response to these items.
Please address any additional questions that you might have to this office.

[4 ATTACHMENT A Item 1 - Infraction 10 CFR 50, Appendix B, Criterion VI states, in part " Measures shall be established to control the issuance of documents... including changes thereto...."
Commonwealth Edison Company Topical Report CE-1-A, Revision 9, Section 6, states in part, "A document control s'ystem'will be used to assure that documents...are reviewed for adequacy and approved for release by authorized personnel... changes to these documents will be handled similarly...."

noncontrolled Observation Memo No. 53, dated April 25, 1979, rather than in accordance with the established document control system.
- b.
Concrete expansion anchors had been identified as nonconforming with Standard BY/BR-CEA, which had been issued after the installation of the anchors.
However, the nonconforming situations had not been documented in accordance with the established document control system.
Corrective Action Taken and Results Achieved:
a.
The 90 connectors are being removed from the subject cables.
All temporary backfeed cables containing these connectors have
, hen nonconformance has been noted, a Hatfield been changed.
W will be Electric Company (HECo.) Nonconformance Report (NCR) written instead of an inter-office memo describing the problem, as was the case in this instance, b.
Field Change Request Number 1140 dated September 19, 1979 has been initiated which iden_tifies 85 locations of the use of concrete expansion anchors prior to the issuance of the requirements of BY/BR-CEA.
The engineering review of the Field l
Change Reg'lest has concluded that no unsatisfactory condition exists with the structure.
l Corrective Action To Be Taken to Avoid Further Noncompliance:
t Electrical terminators were informed about Commonwealth Edison Company Standard #59160.
This standard gives proper size connectors for cable to be terminated.
If there is still play in cable and connector it will be filled witn tape.
Hatfield Electric Company personnel when noting a nonconformance in the field that is safety related will write a HECO. NCR.

Item 2 - Infraction 10 CFR 50, Appendix B, Criterion X states, in part "A program for t
inspection of activities affecting quality shall be established and executed...to verify conformance with the documented, instructions, procedures, and drawings...."
Commonwealth Edison Company Topical Report CE-1-A, Revision 9, Section 10, states in part, " Inspection Programs will be established to provide assurance that the quality control surveillance, inspections...are performed...."

not been established to assure that concrete expansion anchor plumbness, edge and spacing, and exposed length were inspected by all site contractors.
Corrective Action Taken And Results Achieved:
An in depth analysis of all concrete expansion anchor installations performed by Hatfield Electric Co. is being accomplished.
The i
ptrpose of this analysis is to determine what documentation is available pertaining to these installations and to categorize this l
data by type and size of installation and also what time period.
l The information obtained from this analysis will be used to formulate a inethodology for identifying and inspecting concrete expansion anchor installations not conforming to the currently

approved CEA specification.
All concrete expansion anchors install'ed prior to the current specification will be evaluated.
Nonconforming installations will be documented and resolved as directed by Commonwealth Edison Company's Station Nuclear Engineering Department.
Corrective Action To Be Taken To Avoid Further Noncompliance:
All site contractors were directed to revise thel; concrete installation procedures to comply with the currert CEA specification (BY/BR-CEA).
These procedures were reviewed and approved by Sargent
& Lundy (S&L).
Concrete expansion anchor installation activity is under surveillance by Commonwealth Edison Company and contractors to l
assure compliance to these revised procedures.
Since January 1, 1980, the site Independent Testing Contractor, Pittsburgh Testing f

This should assure that l
i, all concrete expansion anchors installed since January 1, 1980, comply with the current CEA specification.
Date When Full Compliance Will Be Achieved:
The analysis of Hatfield Electric Company's concrete expansion anchor activity will be completed by March 15, 1980.
Based on the results of this analysis a timetable shall be established for completion of the identification and evaluation process for concrete expansion anchors installed prior to the current specification.
