SUMMARY
OF LICENSEE EVENT REPORTS (LERs) FOR TMI-2 The docket file of LERs for TMI-2 covered the period from May 29, 1974 toMarch 16, 1979.
These reports are required by Section 6.9.1.9b of the Technical Specifications. The seventy-nine events have been categorized based on the primary cause for deviation to the Technical Specification requirement.
There were reported 31 equipment malfunctions,/5 events which required equipment redesign, 17 human errors (operator and technician),

/5 events, which required clarification of operating procedures, and one notification due to vendor reanalysis of small breaks for TMI-2.
E There were events which could be related to some degree to the TMI-2

accident.
Five of these were related to the feedwater system, one concerned inadvertent opening of the pressurizer EMOV, and the remaining four concerned the emergency core cooling systems. Two of the feedwater transients and f
& opening of the EMOV resulted in,the loss of indicated level in the pressurizer.
The opening of the pressurizer EMOV (LER 78-021) was the result of de-energization of the vital bus.
It was noted in the report that the lack of position indication for the EMOV hampered recovery by the operators since the cause of system depressurization could not be determined.
The licensee recommended and implemented a position indicator for the EMOV.
