The following report is submitted in accordance with paragraph 6.7.2.a.4 (page 6-11) of the Technical Specifications.
On 25 June High Pressure Injection Valve !U-V16A, failed to open on signal from the control room console push-button.
While in the process of restoring the plant to nonral hot shutdown conditions, following the loss of off-site paner test, additional cake-up water from the make-up tank was needed to raise pressurizer level from 85" to 100".
'In order to raise pressurizer level, the High Pressure Infection Valve essociated with the operating 1A cate-up ptap (fU-Vl6A), was signaled to open by use of the control room console mnual push-button. The valve failed'to open.
When l"J-VlGA failed to open, !?J-V16B was imediately opened and ' pressurizer level was restored to 103".
~ Redundant High Pressure Injectiott Valves (ID-V16C and D) were subsequently tested satisfactorily.
