This tele (Region I 'iRC)gran is to confim the conversation between "r. A. ii. Fasano and Mr. J. P. O'Hanlon (T"I-I Unit Supeeintendent of Technical Saccort) at 1600 bcurs on 10 l'iy 1977.

A violation o' T.S. 3.6, Resctor Building Containrent, in that both doors i
e' the Peactor Building Erergency Personnel Access patch were open at the sa e l
tire. The Reactor Coolant Syste was at 3C00F and 390 osig.
Uoon opening the inner door of the E e'rgency personnel Access Hat:h, it aas discovered that the outer door was not fully closed. Irr ed'3tely, the inner dcor was cicsed and the Shi't Fore an was notified.
Subseouent investiqation revealed that the outer deor closing - echanis".
Was in its closed position, thus satisfyino the echanical door interlock. The doer itselwas oren just 'ar enounh 'or the latching her to pass under the deor rcilers rather than over them.
