Gentlemen:

In response to the Item of Noncompliance included in the subject report, the following is provided.

NOTICE OF VIOLATION Based on the results of an NRC inspection conducted during the period May 7-30, 1980, it appears that ertain of your activities were not con-ducted in full compliance with NRC regulations and the conditions of your license, NPF-6 as indicated below:

The licensee failed to adhere to the procedures established in Procedures No. 1005.04, Revision 3, " Control and Use cf Bypasses and Jumpers," in tha t:

Entries 2/79/134 to 2/79/142 and 2/79/142 and 2/79/148 to 2/79/151 made in October 1979, had not been cleared in the Jumper and Bypass Log after the jumper / bypass had been removed, as required in Steps 6.1.6 and 6.1.7, l

Duplicate Tag Numbers, 2/79/148 to 2/79/151 were issued for two separate tagouts, (System 58 and System 65) which is prohibited in Step 4.4, and c.

Completed pages of the Control Room Jumper and Bypass Log had not been forwarded to the Nuclear Plant Administrator for filing as required in Step 7.1.

This is an infraction (368/80-09-01).

RESPONSE

The entries 2/79/134 to 2/79/142 were cleared on 6-2-80.

The entries 2/79/148 to 2/79/151 (System #58) were cleared 6-26-80.

To avoid further non-compliance I&C personnel have reviewed the Jumper and Bypass Procedure with particular attention to sections 4.4, 6.1.6, 6.1.7.

Completed pages of the Jumper and Bypass Log have been removed and forwarded to files.

Full compliance was achieved on 6/26/80.
