Enclosed please find copies of American Warming &

Ventilating, Inc., Customer Notifications concerning the subject 10 CFR 21 for AW Job No's: 80290, 90083, 90084, 90096, 90120, 90121, 90162, 90163, 90181, 90186, 90193, 90216, 90235 and 90236.

Please be advised that:

1) AWV jobs 90096 and 90216 are classified as Non-Nuclear Safety Related Dampers. Consequently, it is AWV's o" pinion that these dampers do noc fall under-the reporting requirements of 10 CFR 21 and as such are removed from AWV's list of affected Customers and Projects, (Attachment to February 6,1980 Notifi-cation and revised March 19, 1980), revised copy enclosed.
2) AW is retract;ing all reference to AWV jobs 90084, 90120, 90121, and 90263 concerning the subject 10 CFR 21. In these instances the ITT-GC Actuators were provided by the Purchaser to AW for in-stallation on to dampers sold. As such, the Purchaser has the responsibility to provide ITT-GC Actuators which meet the torque requirements of the damper and/or subject specification. These jobs are removed from AW's list of Affected Customers and Projects, as described in 1).

This letter is to advise you that AWV has determined that the dampers supplied on this job, reference (1) , are NON-NUCLEAR SAFETY related danpers. Consequently, it is ARV's opinion that these dampers do not fall under the reporting requirements of 10 CFR 21 and has under separate cover so notified the U.S. N. clear Regulatory Commission. AWV has made an evaluation of all ITT actuators shipped against this job (see Ref. 4 fcr Tag Numbers), and all eight (8) of the dampers incorporate ITT actuators yhich neet the torque requirements of the damper and/or your specification. ". . . at the 80% confidence leval, 98% of the units will have outputs at or in excess of the statistica,1 minimum. In a practical sense the statistical minimum of the output forces should represent a number that can be compared to the
  • desired or required actuator output including ' '

safetv factors which were calculated during the design phase. This should allow a reasonable judgement to be made concerning the capability of the actuator to perform as expected." ITT General Controls has advised that their Engineering Department is preparing a complete report for the basis of the data supplied. This report will be sent on to you within sev'en

'7) working days of its receipt. AWV should be receiving this report within the next few weeks. - (1), is NOT affected by the December 7, 1979, ITT-CC reported use of springs having incorrect material as noted in the United States Nuclear Regulatory Commission Office of Inspection an'd Enforcement, IE Bulletin No. 80-09, dated April 17, 1980. The subject springs were identified as No. 1 springs of which none were used on this job.

Also, please be advised that any ITT actuated dampers shipped from AWV since May 1, 1980 incorporate actuators which have been evaluated by either statistical data, actual test or damper test and meet the torque requirements of the dr:per and/or your specification. You will be kept advised of future developments concerning this problem.

Dampers 2HVK-MOD 21A, 21B incorporate ITT actuators , which provide 143% of the required damper torque based , on the statistical minimums and 156% of the required i damper torque based on the means of the actual test i data at the worst condition; fail closed spring end. In the power direction, worse case, the actuator produces 410% of the required damper torque based on the statistical minimums. This evaluation was based on ITT General Controls supplied

statistical minimums and means of actual actuator output forces.

the actuator test rample consisted of eighty (80) actuators.

Statistically, ITT General Controls has stated that

This letter is to advise you that ANV has made an evaluation of all ITT actuators shipped against this job; see the attached listing for the results. This evaluatic-was based on ITT General Controls supplied statistical minimums of actual actuator output forces. The actuator test sample consisted of eighty (80) actuators. Statistically, ITT General Controls has stated that:

Note, the statistical data includes the lowe r minimum force value exerted by a number one (1) spring manufactured from an incorrect material. ITT General Controls maintains that once such a number one (1) spring is co= pressed it will thereafter consistently exert the same force level without degredation. ITT General Controls has advised that their Engineering Department is preparing a complete report for the basis of the data supplied. This report will be sent on to you within seven (7) working days of its receipt. AWV should be receiving this report within the next few weeks. Prior to AWV taking any corrective action on this matter, the ITT General Controls Engineering' Report must be accepted and approved; the data used for this evaluation must be considered preliminary in the meantime. Also, please be advised that all ITT actuated dampers shipped from AWV since May 1, 1980 incorporate actuators which have been evaluated be either statistical data, actual test or damper test and meet the torque requirements of the damper and/or your specification.

Parikh notification of the existence of a possib: defect per 10 CFR 21 - #1 spring defect, da,ted January 8, 1980 (4) U.S. Regulatory Commission IE Bulletin No. 80-09, daded April 17, 1980 Gentlemen: This latter is to advise you that AWV has made an evaluation of all ITT actuators shipped against this job; see the attached listing for the results. This evaluation was based on ITT General Contr.ols supplied statistical minimums of actual actuator output forces. The actuator test sample consisted of eighty (80) actuators. Statistically, ITT General Controls has stated that:

". . .at the 6 1% confidence level, 98% of the units will have cutputs at or in excess of the statistical minimu2... In a practical sense the statistical minimum of the output forces should represent a number that can be compared to the desired or required actuator output including safety factors which were calculated during the P. This should al~ low a reasonable judgement to be made concerning.the capability of the actuator to perform as expected-Note, the statistical. data' include's the lowe'r minimum force value exerted by a number one (1) spring manufactured from an incorrect material; reference 3.
