Description of Incident: The weekly grab sample for oil and grease collected, 6/30/80 indicated 26.2 mg/l oil.

Apparent Cause: Failure of F20/M13 filter elements.

Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence: The suspect filters were replaced on 6/30/80.
