- 8. Corrective Action Preliminary physical examinations were made by SBF on samples of fasteners from each of five (5) heat treatment lots furnished to NPSI and forwarded to the Midland project. Two (2) heat treat-ment lots were revealed to be defective. NPSI has determined that no other SBF fasteners have been procured and/or supplied by NPSI to any other facilities.
Bechtel - Midland was notified of the defect and the intent of NPSI to file a report on June 13, 1980. NPSI and SBF will continue the investigatiun to 1); Assure that all defective fasteners have been identified and 2)J Attempt to determine the cause for the defective material. Dear Don:
Please find enclosed a summary of our investigation to date surrounding all the bolting materials we furnished you (NPSI P.O. PDX 9461, 9478N, 9 541N, and 9542N), which subsecuently went to the Bechtel Midland Project. I believe that we covered most all of the data contained in the summary during our con-ference call yesterday, June 10, 1980. To date, however, our investigation of all materials supplied to NPSI is still incomplete, pending the results of
-_ chemical analyses on samples of the_ returned materials and other ma te rials . We are having that portion of' the work done by an -
outside test lab, and those results are not expected until some time next week. It will be very necessary for us to receive those results in order to confirm the cause of the problem, which now has been limited to SBF.HT, Lot.No. 36 and No. 359. (See attached summary.) Our metallurgical lab is still ana'lyzing samples of other materials that were treated in HT. Lots No. 36 and No. - 359 in order to verify their heat treated condition, etc. Should we determine by such action that any of those materials (not supplied to NPSI) could have also been affected by this -~ particular problem, we will promptly notify the customers that we - know received the material. Representative samples of all materials heat treated in C J Z.1.2 C lo.ts'Non.36;andzNo.a359.twil11 ultimately ~7berre-verifie~d_by1Southsi'nr2 l Bolt during the course of this invest-igation. June 11, 1980 Upon reviewin'g the procurement documents from NPSI to Southern Bolt, we do not believe that they clearly specify that the provisions of 10CFR21 apply. (P.O. No. PDX 9461, PDX 9478N, PDX'9541N, and PDX 9542N). However, based'on our metallurgical lab and O. A'. . Department's findings this week, we believe that we do have a responsibility to advise you that all materials
. supplied to NPSI (Heat Treated in SBF HT. Lot No. 36 and 359) described by our MTR's 6374, 6373, 6372. 6370, 6371, and 5795 may not be in' compliance with the specified recuirements of NPSI Purchase Order, and all material should be returned to Southern
, Bolt for immediate replacement at no expense to NPSI. Materials i covered-bnly by the described MTR's should be returned, unless otherwise instructed by Southern Bolt, at a later date. Our investigation is expected to be completed by June 24, 1980, (earlier if possible).
