Docket No. 50-382/80-07 Appendix A NOTICE OF VIOLATION l

Based on the results of the NRC investigation conducted on April 2-4, 9-11 and May 2, 1980, it appears that certain of your activities were not conducted in full compliance with the conditions of your NRC Construction Permit No. CPPR-103 as indicated below:

Failure to Verify Conformance With Documented Drawings Criterion X of 10 CFR 50, requires that a program be executed to verify conformance with documented instructions, procedures and drawings.

Criterion VII requires control over purchased materials to include l

objective evidence of quality furnished by the contractor, inspection at tl.t material source, and examination of materials upon delivery.

Instructions, as shown on IEW Drawing No. 2662Q-ll4, Revision 2, specified that all welds were to be produced in accordance with American Welding Society (AWS) requirements and inspected to ASME Boiler & Pressure Vessel Code, Section NF requirements.

Contrary to the above:

Eigi.' cold-leg support components ("D" stops) for the reactor coolant piping were furnished by Industrial Engineering Works (IEW) with weld defects prohibited by the noted drawing and specifications.

These supports had been accepted by IEW source inspection and by site receiving inspection without identification of the observed defects.

These defects were incomplete welds, missing welds and misoriented pieces which should have been sufficient for rejection at both source and receiving inspections.

This is an infraction.
