Unit Identification and Description of Occurrence:

While running S.I. 4.5.E.1.d & e,the HPCI failed to meet the flow requirements due to the inadvertatt tripping of the turbine stop valve.

1 Conditions Prior to Occurrence:

Unit 1 - 80% steady state power.

l Unit 2 - 96% nteady state power.

Unit 3 - 100% steady state power.

Action specified in the Technical Specification Surveillance tiequirements met lLue to inoperable equipment.

Describe.

RCIC, Core spray, ADS logic, and RHR (LPCI) systems were proven operable.

api!,arent Cause of Occurrence:

Hydraulic pressure sufficient to open the stop valve did not develop due to_a worn mechanical overspeed trip mechanism piston.

Analysis of Occurrence:

There was no danger to health or safety of the public, no release of activity, no damago i
