We have reviewed your letter of January 6, 1975 and the following is our response to the apparent violation cited in that report. This response details the violation, corrective action and results achieved, steps taken to avoid further violation and the date when compliance will be achieved.

The apparent violation consisted of the lack of completion of a page of the Jumper and Bypass Log as required by Procedure 1005.04, " Control and Use of Jumpers and Bypasses". The particular discrepancies noted were corrected immediately. To assure that no further violations of this type occur, the procedure for the use of jumpers and bypasses was discussed in detail with supervisory personnel. The need for strict adherence to the procedure was emphasized. Also, a training session is being planned for non-supervisory personnel.

Sessions for affected operations personnel are to be complete by March 1,1975. Sessions for affected maintenance and instrument personnel are to be completed by February 1, 1975. The details, as well as the importance of strict adherence to the procedure, is to be emphasized.

We feel that this effort should be quite adequate to assure no further violations of this nature.
