Description of Occurrence:
At 1515 hrs. on 9/26/76, leaks were discovered at a weld on cooler E27B's outlet nozzle and in the pipiag upstream of valve SF-1.
The Icak on the outlet nozzle of E27B was thought to have been caused by a pin hole in the weld but was discovered to have been caused by three cracks in the nozzle itself. The leaks upstream of valve SF-1 were caused by two cracks in the heat affected zone above SF-1.
The cracks in the nozzle were 1 1/2" to 2" long. The cracks in the pipe were 1" to 1 1/2" long.

X The cause of the failures in the nozzle is unknown. The cause or the failures in the schedule 10 piping is similar to previous failures in schedule 10 piping with heat number 800201.

Analysis of Occurrence:
The leaks were minor and were contained in the Reactor Auxiliary Building; hence, there was no hazard to the health and safety of the public.

Corrective Action:
The nozzle for E27B will be replaced.

The ten inch schedule 10 pining was replaced with a six inch section between the elbow a..d SU-1.

Failure Data:

This is the 7th similar occurrence in the vicinity of a weld on schedule 10 piping. The apparent mode of failure is the same as outlined in 50-313/74-118.
