8. Designation of Apparent Cause of Occurrence:

Design Procedure .

s] - Manufacture Unusual Service Condition Including L Installation / Environmental Construction Component Failure Operator Other (Specify) The apparent cause was due to a decrease of lake water temperature caused by cold weather.

9. Analysis of Occurrence:

1 The only noticeable Environmental Impact was the removal of the ab61 mentioned fish from the lake. The fish that were collected were dis posed of through landfill operation. The fish that were not collect were disposed of through the trash disposal system.

10. Corrective Action:

I Long term studies are in progress to determine the effects of plant olieration on the aquatic ecosystems of the Dardanelle Reservoir. D:

from this report and other data obtained is fed back into these studies to determine what changes might be made to the studies in order to ensure that adequate data is obtained for evaluating plant [

impact. Review by APSL and its consultants of the results of these' I studies will determine what corrective action might be taken to redi plant effects on the Reservoir.
