10 CFR 50, Appendix B, Criterion V, states that, " Activities affecting quality shall be prescribed by documented instruction, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures or drawings.
Contrary to the above, the contractor providing maintenarce services on the emergency diesel generator during the current refueling outage had not been evaluated and approved as a qualified supplier of such services by your Quality Assurance organization in accordance with the pro-visions of facility Quality Assurance Procedure 5, Rev.1, " Supplier Quality Assurance," which states, in part, that " Suppliers evaluations for services, material, and equipment contracts will be made by the Quality Assurance Director or his designee using the required portion of appli-cable forms prior to the contract ~ award."
