DAVIS-BESSE UNIT ONE NUCLEAR POWER STATION SUPPLEMENTAL INFORMATION FCR LER NP-33-77-47 DATE OF EVENT: August 13, 1977 FACILITY: Davis-Besse Unit 1 .

IDENTIFICATION OF OCCURRENCE: Loss of Shield Building Integrity due to Construction personnel failing to latch door after entry Conditions Prior to Occurrence: The plant was in Mode 2, with Power (MWT) = 0 and Lead (MWE) = 0.

Description of Occurrence: At 1045 hours0.0121 days <br />0.29 hours <br />0.00173 weeks <br />3.976225e-4 months <br /> on August 13, 1977 while perfor=ing ST 5061.06, Shield Building Integrity Monthly Test, the access door to the Detergent Waste Drain Tank Room (Door 108) was discovered to be open and unlatched.

This placed the Unit in the Action Statenent of Technical Specification 3.6.5.2.

Designation of Apparent Cause of Occurrence: Construction workers had entered 3

j through the door into the Detergent We, e Drain Tank Room and had failed to close u/ the door after entry.

Analvsis of Occurrence: The loss of shield building integrity did not pose a threat to the health and safety of the public or to station personnel. An accident requir-ing shield building integrity did not occur.

Corrective Action: Immediately the door was closed and latched, and the Construction workers were informed that these negative pressure boundary doors .

nust remain closed. Tigns have been installed on these doors stating that they nust remain closed except during nor=al transit entry and exit.

Failure Data: Loss of Shield Building Integrity has occurred on two prior events (NP-33-77-32 and NP-33-77-33). One event was due to a deliberate entry to per-form a surveillance test, and the other was due to construction cables being hung through the doorways, b)
