Toledo Edison acknowledges receipt of your June 28, 1978 letter and enclosures, Appendix A and Report 78-13, referencing an apparent deviation from Davis-Besse

in Appendix A.

Infraction: Technical Spec!fication 6.8.1 requires that written procedures shall be established, implemented, and malatained.

Contrary to the above:

1.

The performance of requirements of ST 5013.03 on March 30, 1978 were not properly documented.

2.

The performance of requirements of MI IL-1701 used in asso-ciation with maintenance work order I&C 753-77 were not properly documented.

3.

Shift foremen were not recording tests completed to demon-strate post maintenance operability on maintenance work orders per the requirements of AD 1844.00.

Examples are MWO's I&C 277-78, I&C 738-77, and I&C 77-0973.

4.

AD 1838.02 requires the shift foremen to review completed surveillance test and take any action required to meet Technical Specification requirements. On ST 5031.14 per-formed November 1, 1977, the shift foremen failed to note the test deficiency and take the required Technical Speci-fication action.

Response

In each of these cases above, personnel were not following approved management controls. The individuals involved with each of these four cases have been informed of the seriousness of events of this nature. In addition, designated reviewers have been informed of the importance of a proper review of surveillance test results and AD 1844, Maintenance will be revised by July 30, 1978 to more clearly define the applica-bility of post maintenance operability.

Generically, we feel our management systems are adequate and we will continue to insure their implementation. This matter has attention and involvement at the corporate vice-presidential level.

All station personnel have been informed of the importance of

addition, the need for adherence to procedures was addressed in our training program. However, in view of the present situation, we have established a program to place a greater emphasis on adherence to procedures in our training program.

Management has requested that all failures, to properly implement management controls, be immediately brought to their attention for corrective action.
