TOLEDO EDISON COMPANY Q DAVIS-BESSE UNIT ONE NUCLEAR POWER STATION SUPPLEME'1TAL INFORMATION FOR LER NP-33-77-53 DATE OF EVENT: August 4,1977 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Auxiliary Feed Pump 1-2 inoperable due to ground in speed switch SS 816.

Conditions Prior to Occurrence: The plant was in Mode 3, with Power (MWT) = 0 and Load (MWE) = 0 Description of Occurrence: On August 4, 1977, at 1420 hours0.0164 days <br />0.394 hours <br />0.00235 weeks <br />5.4031e-4 months <br /> while Operations per-sonnel were running Auxiliary Feed Pump (AFP) Turbine 1-2, control of discharge valve AF 388 was lost. This placed the station into the Action Statement of Tech-nical Specification 3.7.1.2 which requires both Auxiliary Feed Pumps to be operable in Mode 3, Hot Standby.

A Electricians investigated and found the control power fuses blown to the valve.

l 1 They replaced the fuses and Operations personnel stroked the valve. The fuses

\s /# blev again. Power was then removed from the valve and AF 388 was manually opened at 1530 hours0.0177 days <br />0.425 hours <br />0.00253 weeks <br />5.82165e-4 months <br /> on August 4,1977. This returned AFP l-2 to operability and removed the station from the Action Statement of Technical Specification 3.7.1.2.

Designation of Apoarent Cause of Occurrence: The cause of this occurrence was determined to be a ground on the terminal board of Auxiliary Feed Pump Turbine 1-2's speed switch, SS-816. This switch will open the valve when the speed reaches approxi-mately 2800 RPM. The ground was caused by a crack in the terminal board.

Analysis of Occurrence: Since AFP l-1 was operable, there was no danger to the health and safety of the public or station personnel.

Corrective Action: A work order (IC-434-77) was issued, and the Instru=ent and I Control Section temporarily repaired the ground on August 5,1977. Operations per-sonnel then successfully stroked the valve three times using the speed switch. A second work order (IC-501-77) was issued to replace the faulty terminal board and to inspect the terminal board on AFP l-1.

Failure Data: No prior events have be'en caused br a failure of a terminal board.
