T0lEDO EDISON COMPANY DAVIS-BESSE UNIT ONE NUCLEAR POWER STATION SUPPLEMENTAL INFORMATION FOR LER NP-33-77-35 DATE OF EVENT: July 24, 1977 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Containment Personnel Air Lock was rendered in-operable and Containment integrity was also violated.

l Conditions Prior to occurrence: The plant was in Mode 4, with Power (MWT) = 0 and Load (MWE) = 0.

Description of Occurrence: At 0920 hours0.0106 days <br />0.256 hours <br />0.00152 weeks <br />3.5006e-4 months <br /> on July 24, 1977, the personnel lock was rendered inoperable when Maintenance personnel defeated the mechanical door interlock during inspection of the Camrol in the exterior door =achanis=

on the personnel lock. This action placed the station in the Action State =ent of Technical Specification 3. 6.1.3.

While the interlock was defeated and the exterior door was open, personnel inside Containment opened the interior door to exit Containment which violated Contain-s- ment integrity. This placed the station in the Action Statement of Technical Specification 3.6.1.1.

Designation of Accarent Cause of Occurrence: The defeating of the interlock to render the personnel lock inoperable was a planned action to allow Maintenance personnel to inspect the Camrol in the exterior door =echanism.

The violation of Containment integrity was due to personnel error in failure to rope off and/or place "D0 NOT OPER ATE" tags on both the exterior and interior doors.

4 Analysis of Occurrence: Since initial criticality had not yet been obtained, there was no threat to the health and safety of the public or to station person-nel.

Corrective Action: At 0921 hours0.0107 days <br />0.256 hours <br />0.00152 weeks <br />3.504405e-4 months <br /> on July 24, 1977, Containment integrity was obtained when the interior door was closed. D0 NOT OPERATE" tags were placed on the exterior and interier doors to prevent recurrence while the interlock was defeated.

s At 1330 hours0.0154 days <br />0.369 hours <br />0.0022 weeks <br />5.06065e-4 months <br />, the interlock was restored to service and the personnel lock was i declared operable removing the station from the Action Statement of Technical s_- / Specifications 3.6.1.1 and 3.6.1.3.

Maintenance personnel have been instructed to notify the Shift Foreman prior to defeating the interlocks and to rope off and/or place "D0 NOT OPERATE" tags on the interior and exterior doors when interlocks are defeated, l Failure Data: The Containment Personnel Air Lock was previously declared in-operable on July 3, 1977, (NP-33-77-18), but the cause was fabrication error rather than a planned action.
