Marble Hill Nuclear Generating Station - Units 1 and 2 In accordance with the provisions of Section 2.201 of the NRC's " Rules of Practice", Part 2, Title 10, Code of Federal Regulat' ions, Public Service Company of Indiana, Inc. (PSI) offers the following information, in addition to PSI's letters (DSN 0411790038, 0625790073, 060790093 and 0706790093) dated April 10, 1979, June 25, 1979, June 7, 1979 and July 6, 1979, respectively, in response to the items of noncompliance identified in inspection report numbers: 50-546/79-02; 50-547/79-02 and 50-546/79-03; 50-547/79-03 and 50-546/79-04; 50-547/79-04.

The purpose of this letter is to provide an updated status as these items have been either addressed in subsequent inspection reports or the corrective action has been delayed or changed due to the NRC Confirming Order of August 15, 1979.

Item of Noncompliance (546/79-02-02; 547/79-02-02)

Special storage instructions for the storing of safety-related pumps and motors did not exist.

Corrective Action Taken and Results Achieved Previously addressed in PSI letter (DSN 0411790038), dated April 10, 1979.

' Corrective Action Taken to Prevent Recurrence Review of documentation and procedures indicate that full compliance has not yet been achieved. This item has subsequently been addressed in NRC reports 50-546/79-25; 50-547/79-25 and 50-546/80-03; 50-547/80-03 with corrective actions delineated in PSI letters (DSN 0226805026, 0329806512, 0331806505 and 0411806512) dated February 15, 1980, March 10, 1980 and April 11, 1980, respectively.

D As explatned in the above referenced letters, PMP 6.03 which deals with

  • storage of safety related equipment has been issued and is being implemented.

Date When Full Compliance will be Achieved As explained in the above referenced letters, full compliance will be achieved with the full implementation of the Material Management Plan scheduled for July 15, 1980.

Item of Noncompliance (546/79-03-03; 547/79-03-03; 546/79-04-02; and 547/79-04-02)

Contrary to the requirements of 10 CFR 50, Appendix B, Criterion X

" Inspection", it was determined that adequate QC inspection was not being performed (by Newberg-Marble Hill).

Corrective Action Taken and Results Achieved Previously addressed in PSI letter (DSN 0625790073, 060790093 and 0706790093) uated June 15, 1979, June 7, 1979, and July 6, 1979, respectively.

Corrective Action Taken to Prevent Recurrence Review of documentation and procedures indicates that full compliance has not yet been achieved. Newberg revised it's QAPN-10 to include new requirements for concrete pours and better QC coverage of placement activities Rev. 5, November 20, 1979. However, Concrete Placement procedures are presently undergoing further revision as part of an overall revision of the Newberg Quality Assurance Program and procedures as described in PSI letters (DSN 0318806518 and 0502806500) dated March 6, 1980 and May 2, 1980, respectively.

These procedures are being submitted to PS1 for review and approval.

Date When Full Compliance will be Achieved The procedures have been rewritten to further clarify the inspection process.

It is expected that review, revision and approval will be completed on June 16, 1980 as described in PSI letters (DSN 0318806518 and 0502806500) dated

. March 6, 1980 and May 2, 1980, respectively.

Item of Noncompliance (546/79-03-04, 547/79-03-04)

Contrary to the requirements of 10 CFR 50, Appendix B, Criterion II " Quality Assurance Program," it was found that Whalen-Chilstron Joint Venture's (WCJV)

Z: QA/QC Program did not provide for the proper indoctrination and training of

" post-tensioning QC personnel. In addition, WCJV records showed that the qualification certification requirements of ANSI N45.2.6 were not met for the post-tensioning personnel.

Corrective Action Taken and Results Achieved The Qualification Program and Attachment G of WC QAP-7 (Certificate of Qualification) for the post-tensioning embedment inspector have not been updated to include this added responsibility as reported in PSI's letter (DSN 0625790073) dated June 25, 1979.

Correctivi Action Taken to Prevent Recurrence The Qualification Program including Attachment G of WC QAP-7 (Certificate of Qualification) have been rewritten. This is part of an extensive change to WCJV QA/QC Program. It is presently undergoing review by PSI personnel. This e is part of PSI's overall program review as described in PSI letter dated February 28, 1980 entitled " Description of License Activities Addressing Order Confirming Suspension of Construction".

Date When Full Compliance will be Achieved It is expected that review, revision and approval will be completed on June 16, 1980.
