Dear Dr. Hubbard:
This is in reply to your undated letter, in which you requested, pursuant to the Freedom of Information Act, copies of documents relating to the cost benefit analysis of the Order of May 7,1980, the list of the incidents listed in the first paragraph of Section I of the Order including the estimated excess exposures received by both patients and radiation therapy personnel, and the way in which the indicated malfunction was detected.
With respect to your first request, no cost benefit analysis was performed.
Concerning your second request, a list of incidents and related exposures, which were known to the NRC at the time the Order was issued, is enclosed.
With respect to your third request, as to how the indicated malfunction was detected, this information is not readily available in our records.
