APPENDIX A NOTICE OF VIOLATION Tennessee Valley Authority License No. DPR-68 Browns Ferry Based on the NRC inspection February 4 through March 7, 1980, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspon-dence to you dated December 31, 1974.
As xequired by Technical Specification 3.5.F.2, the Reactor Core Isolation Coolant System (RCICS) may be inoperable for a period not to exceed 7 days if the High Pressure Coolant Injection System (HPCIS) is operable during such time period.
In addition, Browns Ferry Standard Practice 12.2 requires the Shift Engineer, Assistant Shift Engineer and Unit Operator to keep journals and these journals shall contain significant equipment malfunctions.
Contrary to the above, on December 8,1979, RCICS was inoperable for approx-imately 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> for replacement and calibration of the Electro Governor-Mechanical (EGM) box and HPCIS was not determined to be operable.
In addition, neither the Unit Operator, Assistant Shift Engineer nor the Shift Engineer documented in their jou csls that RCICS was inoperable for replace-ment and calibration of the EGM box.
This is an Infraction.
