O, STN 50-482/80-05 Appendix A NOTICE OF VIOLATION Based on the results of the NRC inspection conducted curing March 17-20, 1980, it appears that certain of your activities were not conducted in full compliance with the conditions of your NRC Construction Permit No. CPPR-147 as indicated below:
Failure to Provide Prompt and Adequate Corrective Action 10 CFR 50, Appendix B, Criterion XVI requires that conditions adverse to quality be promptly identified and corrected.
SNUPPS Quality Assurance Program, Wolf Creek Site Addendum, Section 17.1.16(b) requires that the cause of the nonconformance be determined and prompt corrective action taken to preclude recurrence of those significant conditions adverse to quality.
Daniel RMI-W-105(Q), Revision 3, " Receiving and Maintenance Instructico for Reactor Vessel Head," and RMI-W-090(Q), Revision 1, " Receiving and Maintenance Instruction for Reactor Internals," specify that Level C storage (modified temperature and humidity requirements) be given to the reactor vessel head and reactor internals.
Contrary to the above:
On March 19, 1980, the IE inspector found water standing on the floor of the storage area for the reactor closure head and reactor upper and lower internals.
Review of the Daniel QC surveillance reports
- ter-e mined that this ingress of water had been a continuous subject of deficiencies for thirteen months.
QC storage Surveillance Reports No.
052, 475, 674, 885, 949, 1057 and 1189; Deficiency Reports ISD2807 and ISD1471; and Corrective Action Report 1-G-003 document presence of water in the reactor head and internals storage area periodically from February 1979 through January 1980.
These documents also indicate that the roof had been replaced and wall seams were foamed in an' attempt to preclude the entry of water into the storage area. The deficiency tracking documents were closed indicating the satisfactory implementation of corrective action.
However, the measures taken to make the walls and roof weather tight were inadequate and the continued presence of standing water in the storage area indicates that corrective action taken by management has not been prompt nor adequate to preclude recurrence of these conditions which are adverse to quality.

The subject of storage of the reactor vessel closure head and upper and lower internals had been discussed with licensee representatives in April 1979 (Inspection Report STN 50-482/79-07) and was the subject of an item of noncompliance (Inspection Report STN 50-482/79-09).
This is an infraction.
