l NOTICE OF VIOLATION Sacramento Municipal Utility District Sacramento, California Docket No. 50-312 Based on the results of an NRC inspection conducted on June 4, 10-12, 1975, it appears that one of your activities was not conducted in full compliance with conditions of your NRC Facility License No. DPR-54 as indicated below.
Contrary to Technical Specification 6.8.1, approved procedure AP 26,
" Abnormal Tag Procedure," was not adhered to for the control of jumpers and bypasses installed during the operation and maintenance of safety related equipment on components. The procedural requirement that the shift supervisor sign each abnormal tag prior to issuance and retain the stub from the tag had not been implemented.
O This infraction had the potential for causing or contributing to an occurrence related to safety.
Another infraction, identified through your management control system, which was reported as Abnormal Occurrence Report No. 75-9 in a timely manner and corrected, is described in the attached inspection report.
