Dear Sir:
As defined by Technical Specifications for the Fancho Seco Nuclear Power Station, Section 6.12.2B, we are reporting the follcwirg abnonral occurrence as A0-74-10.
At 1230 on October 30, 1974, a report was received in the control rocm that(the A=iliary Feedwater Pt=p "B" (P-319) was emitting large quantities of sncke.) Le pt='s was being used to transfer unter from
)
the condensate storage tank to the steam generatcrs du"ing a feed and bleed operation to irprove the water chemistry in the steam generators.
s 2e pump was shut down inmediately upon receiving the report. The plant was at cold shutdown conditions and no transient was associated with this occurrence.
Tne Auxiliary Feedwater Pump was inspected and the lead packing was fourd melted and leaping out of the glands. Tne ccucling to the notor was renoved and nnnual rotation of the shaft was unsuccessful.
When disrantled in the plant shop, the casing wear rings were distorted and frozen to the impeller, the seal rings were frozen to the bushings and the bushings were frozen to the drive shaft. !!ew wear rings, bushings and a new shaft were fabricated at the shop, the seven ircellers were inspected and reused ard the complete pump was reasse~ bled to factcry specifications. The ptz:p was installed ard tested on Nove=ber 11, 1974, and met all the Technical Specification ecuirerants.
The Plant Eeview Cennittee review on Move-ber 1,1974, concluded
. that an cperator e"rcr xhich failed to cpen a suction "alve between the cendensate storap tank and the punr su:tien was the cause of the failure.
Le feed and bleed cperation has been a routine Ope"ation and ne procedure has been developed. Ee Cca:nittee concluded that a procedura to feed ard 8002210 12GG5
