50-312 Ltcense No. OPR-54 NOTICE OF VIOLATION Based on the results of an NRC inspectio.: conducted on dene 13-16 and 24, 1977, it appears that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:

Appendix B, Technical Specification 4.10, requires the installation of TLD badges at specified locations in the environs of the facility to measure integrated radiation levels. Badges are to be changed at quarterly intervals.

Contrary to this requirement to measure radiation levels in the O

environs of the facility, your processing of the TLD badges for the first quarter of 1977 was performed with a defective instrument resulting in invalid environmental radiation exposure data for that quarter.

This is an infraction.

Appendix A, Technical Specification 6.11, requires that procedures for personnel radiation protection shall be prepared consistent I

with the requirements cf 10 CFR 20 and shall be approved, maintained and adhered to for all operations involving personnel radiation exposure. Rancho Seco Procedure AP-500, " Emergency Plan,"Section III.E, " Radiation Evacuation Procedures," statas that "a sinale unannounced constant air monitor radiation alarm" will be causa for an in-plant local area evacuation to be initiated. This procedure also states that "in cases where the alarm is associated with an air monitor, appropriate respiratory protection will.be worn when initial re-entry and alarm verification is made."

' Contrary to this requirement, on April 14, 1977, an evacuation of the waste gas compression room area of the auxiliary building was not initiated until 44 minutes after the VSG airborne activity monitor reached an alarm point for the waste gas compressor room.

In addition, initial verification entries into the high airborne radioactivity area were made by an auxiliary operator and a chem-ical/ radiation assistant without wearing respiratory protection.

This is an infraction.
