\\d is hereby reporting the following Reportable Occurrence as R0 76-6.
On May 1, 1976, the Reactor Building Polar Crane passed over the reactor vessel while carrying the crane lifting tripod. The i
reactor was in a shutdown condition, the vessel head was removed t
and no refueling operations were being conducted.
This action violated the requirements of Technical Specifications Section 3.11.3 which prohibits lifting of heavy loads over irradiated fuel, excepting components that must be moved during refueling.
Reportable Occurrence 76-7, reported to the Commission on April 27, 1976, was concerned with violation of another part of this specification caused by lack of approved procedures for making lifts over the Fuel Transfer Canal sith the same plant conditions. The corrective action instituted for that occurrence was the generation

of a procedure governing operation of the polar crane. This procedure set up a restricted area around the vessel, which was violated by the 2:
movement of the tripod.

~ lift and the crane operators were not then aware of all of its restrictions.

To assure that all crane operators follow the procedural requirements of g
the crane operating procedure, a formal training session will be attended by crane operators prior to their operating the polar crane. This training session will be-repeated as required as part of the maintenance training program.
4 There was no plant transient associated with this occurrence.
