Abnormal Occurrence as A0 75-17.

j Review of inspection reports on hydraulic sway suppressors (snubbers) following the scheduled 30-day and 120-day surveillances in November, 1975, revealed certain apparent deficiencies in those and previous inspections.

An Internal Audit Report filed November 26, 1975, stated that some of the inspections required by Technical Specifications Section 4.14 were not per-),'.

formed in accordance with the approved surveillance procedure. Indications, of these deficiencies included notations made on the surveillance form identifying several-inaccessible snubbers, although those snubbers had been marked acceptable on previous inspections, improper identification of-several snubbers on the ir.spection form which should have been discovered and reported, and tape found on several anubbers which should have been

-removed during earlier inspections.

When it became evident that problems existed with earlier inspections extra effort was made to assure the adequacy of the November inspections before their approval. None of the snubbers evidenced signs of failure, and the oil reservoirs were filled to their proper level. Only one snubber was found with an abnormally low oil reservoir icvel, which indicates no significant reduction in snubber effectiveness.

. This occurrence is a failure of personnel to perform surveillance in accordance with a written procedure. To facilitate compliance with the snubber procedure, it will be revised to_ require removal of all foreign material :found, all snubbers requiring periodic inspection will be located 33 and marked distinctively, and provisions will be made for proper access to each snubber.

In' addition, to ensure that surveillance procedures are being followed, an audit of 20 percent of those procedures, including the snubber surveillance procedure, will be performed. A representative set of procec. ares selected with respect to group responsibility, type of testing required and systems tested will be witnessed by a foreman or supervisor to assure that the procedures are being followed. Data from previous surveil-

lances will be compared with data gathered in these inspections and tests to reveal discrepancies indicative of procedures not being adhered to.

The audit and any actions that may be taken in response to,the

[h results will assure that surveillance procedures will be followed in the.

future, -preventing recurrence of this event.
