Dear Mr. Delaney,
The Toledo Edison Company and the Cleveland Electric Illuminating Company hereby jointly make application to amend material license SI4-1601 issued June 16, 1976 as follows:
1.
Authorization is requested to receive, store, handle and install seven (7) irradiated surveillance specimen capsules containing by-product material typical of irradiated weld metal and base metal material used in the fabrication of reactor vessels.
Each surveillance capsule is 2.5 inches 0.D., 2 inches I.D. and approximately 24 inches long.
(Reference Topical Report - BAW-10006A) The capsules are part of the Arkansas Nuclear One-Unit 1 and Rancho Seco reactor vessel surveillance programs.
The capsules are to be installed in the surveillance specinen holders installed on the reactor vessel internals of Davis-3 esse Unit 1 prior to initial fuel loading.
Three (3) capsules from Arkansas Nuclear One-Unit 1 (DPR-51 issued 5-21-74) have radiation levels of 700 - 1000 MR at three feet.
The remaining four (4) capsules from Rancho Seco (DPR-54 issued 3-16-74) will have much lower radiation levels.
Radiation protection provisions for the capsules will be under the control of the Davis-Besse station organization described in 2.1.1 of the licensee's application dated November 7, 1975.

Application is-made to register the licensee as a user of the GE Model 1600 container (Certificate of Compliance 9044 Rev. 1) in accordance with the provisions of 10 CFR Part 71.
The above request contains additional information previously unavail-able in connection with section 3.1.2 of the licensee's application dated November 7, 1975. As stated in 3.1.2 this authorization is requested to permit return shipping of the startup sources in their shipping containers to the supplier in case of damage to the sources or decay of sources because of unforeseen delays.
