Dear Mr. Keppler:
In accordance with 10CFR Part 21.21(b), t.his is a report of a defect in i
some electrical terminations in esacntial systems outside enntainment in 1
the Davix-flenze Nucicar Power Station Unit 1.
This defect is similar to i
the defcet reported in Serial 429 dated 4/17/78, and it involves the use of environmentally unqualified "Scotchfil" brand electrical insulating putty and Scotch Brand 33+ electrical tape.
The inside containatenL use of the tape and putty previously ecported caused us to investigatu the ponsible uso nf the tape and putty outside containment-Design review indicated that only the following equipnent was suspect: morn m perated valves, solenoid-operated valves, motors rated at 600 volta and below. Design of other equipment used ternisal j

Two hundred and one devices potentially had spliced terminations utilising environmentally l

Physical investigation of there 201 devices revealed a significant usage of the environments 11y unqualified espc and i

i Radiation is the principle envircumental factor of concern, and the predicted accumulated radiation outside containment over the unit life is very low. The exact failure mode of the tape and putty at low radia-tion. levels is unknown, and slight degradation of the tape and putty t
