This Final Report is submitted in accordance with 10CFR Part 50.55(e) regard-ing the apparent deficiency involving several type SMB Limitorque motor operators furnished for the Davis-Bease Nuclear Power Station Unit No. 1.
This was discussed by Mr. J. D. Lenardson, Quality Assurance Manager, with Mr. D. W. Hayes, NRC Region III Office of Inspection and Enforcement, en April 15, 1977.
Description of the Deficienev The " Installation Tips" section on page 3 of the Limitorque Type SMB Instruc-tion and Maintenance Manual SMBI-170 (BP-5M-9/71) requires that the stem nut be secured tightly by the locking nut and that the top thread of the lock nut be crimped or staked. In February,1977, the Limitorque motor operators on two nuclear safety related valves were found running with the valve stem turning but the valve not opening. Investigation revealed that the lock nuts bad fallen off and the stem nut was loose, thus not operating the valve.
The top thread of the lock nuts had not been staked or crimped as required by the Limitorque Instruction and Maintenance Manual. Further investigation revealed that on 17 of 19 Limitorque operated valves inspected, the top thread of the lock nut had not been crimped or staked. These motor operators were furnished on equipment supplied by Velan, Powell, ITI Hammel-Dahl, and l
Contromatic, who failed to fully implement Limitorque nanual requirements.
l Corrective Action Taken There are 232 Limitorque motor operators installed at Davis-Besse Unit No. 1.
Of these,144 are considered to be nuclear safety related. An inspection program was established and implemented in which all 232 Limitorque motor operators were inspected for staking. Those operators found not to be staked were staked by Toledo Edison maintenance personnel. Of the 144 nuclear safety related operators inspected, 65 required staking while 50 were installed on a type of valve in which staking is not applicable.
