Reactor Building Pressure Transmitter Out-of-Calibration Conditions Prior to Occurrence: Refueling Shutdown Description of Occurrence:

During the annual surveillance testing, on September 8, 9, 1978, Engineered Safeguards (ES) Channels 1 and 3, Reactor Building Pressure Transmitters (PT-4, ~6) were calibrated.

During a review of the calibration data on September 14, 1978, it was determined that both channels had been out of calibration in the non-conservative direction.

Cause of Occurrence:

The apparent cause of the occurrence was extreme temperature and humidity conditions within the penetration room.

These conditions caused the trans-mitter 'zero' point to shift.

A valve, with a body to bonnet leak which caused the extreme conditions, has been replaced.

Analysis of Occurrence:

The calibration data indicated that ES Channel 1 would have tripped at 4.426 psig; lhannel 3 would have tripped at 4.225 psig.

Channel 2 was also oper-able with a trip setpoint of 3.4 psig.

The required ES actuations (HPI, LPI, RB cooling initiation) would have occurred in the event of a postulated accident with no significant delay time.

It is thur, concluded that the health and safety of the public were not endangered.

Corrective Action

The transmitters had been calibrated prior to the determination that the transmitters were non-conservative.
