FINAL REPORT LOOSE KEY IN BETTIS ACTUATOR FOR HENRY PRATT BUTTERFLY VALVES 1.

Description of Deficiency The Henry Pratt Company notified MP&L by letter of the existence of a potential problem with Bettis Robot arm actuators installed on Henry Pratt Butterfly valves which had been furnished to GGNS.

The valve shaft to actuator key could fall out of place rendering the valve inoperable if the valve assembly was installed with the operator below the horizontal axis.

Three containment isolation valves in Unit I have been identified as being installed in such a configuration. These valves are located in the Combustible Gas Control System and the Containment Cooling System.

Unit 2 valves have not been installed.

Safety Implications

~

Had this condition remained uncorrected, it could have adversely affected the safety of operations of the plant by permitting the release of radioactive and combustible gases from the containment.

Corrective Actions The operators that are installed so the key can work loose vill be reoriented if possible.

Should reorientation not be possible, then new keys of longer length will be installed whereby the key is up to the actuator plate and could not slip down if inverted.

A Field Change Request 2111 be issued against the drawings which cover the installation of these valves for Unit 2 to note the proper installation of valves.

Details of corrective action are being tracked by MCAR-GGNS 81.
