Waterford SES Unit No. 3 USNRC Inspection Report 50-382/RPT 80-06

Dear Mr. Seyf rit:

The following information regarding the inf raction cited by the USNRC Inspector s against Tompkins-Beckwith , I n:: . in IE Inspection Report No.

50-382/RPT 80-06 dated April 4,1980 is herewith submitted.

A. Failure to Follow Contreat Procedures Related to Care and Maintenance of Containment Penetration c.ssemblies Criterion V Appendix B 10CFR50 requires that activities af f ecting quality shall be accomplished in accordance with prescribed in st ruc tion s, procedures, and drawings.

Ebasco Procedure CMI #141, " Care and Maintenance Instructions for the Containment Penetration Assemblies", per Paragraph D.2 requires that after being moved to in place storage, the equipment be provided with adequate protection frca damage and deterioration a s a re su l t of conditions or activities in the vicinity. In addition, TBP-46, Revision A, " Care and Maintenance of Permanent Plant Items",

per Paragraph 6.3.1, implements the CMI procedure and requires that surveillances of the installed equipment be performed on a monthly basis.

Contrary to the above:

On March 20, 1980, the IE Inspector cated the following conditions.

j 1. Scaf folding for trades working ia the area of the installed I penetration assemblies was in several places, installed directly against the assemblics.

2. Protective covers for the openings of the assenblies were removed and/or were not installed. Febru:ry 18, 1930 Surveillance Report 730 and Surveillance Report 731 indicated that the openings of the couponents were covered.

3. Piping and piping subassemblies vcre st ored inside some of the 5 penetrations, and in sone cases, were directly rapported by the penetration assenbly. ,g This is an infraction.

Tompkins-Beckwith, Inc. has submitted the following information regarding l the above.

t i 1. Corrective Action Taken and the Results Achieved: Penetration 55 and 69 have been corrected to comply with the requirements of CMI #141.

Piping and piping subassemblies have been removed and each penetration vacuumed of debris and the openings sealed for protection.

Scaffolding has bes n removed from the penetration area.

2. Corrective Action Taken to Preclude Repetition: QC will increase surveillance inspections to ensure that CMI requirements are.being complied with. Inspections will be conducted on a weekly basis and the results documented.
3. Date When Full Corrective Action Uill 3e Achieved: Piping and piping subassemblies were removed on 4-28-30. Debris was removed from subject penetration on 4-24-80. Scaffolding was removed from the pen'etration area during the week of 4-21-80.

QC implemented weekly inspections on 4-24-80. Inspections are presently in process for all other penetration asseablies to ensure CMI requirements are being maintained. Following these inspections, all penetration openings will luve been sealed for protection.
