b Anderson-Greenwood Company DocLet No. 99900214/80-01 NOTICE OF DEVIATION Based on the results of an NRC inspection on March 3-5, 1980, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:

A.

Criterion II of Appendix B to 10 CFR 50 states in part:

"The program shall provide for indoctrination and training of personnel performing activities affecting quality as necessary to assure that suitable proficiency is achieved and maintained."

In response to the above requirement, Section 1.7 of the Quality Assurance Manual states in part:

" Indoctrination and training

.shall be conducted in the various departments under programs.

the responsibility and control of the Director, Quality Assurance.

An outline prepared and approved by the Director, Quality Assurance, specifying the various duties and responsibilities of the individuals shall be used as a guide in conducting the training sessions."

Contrary to the above requirements there was no evidence that the Director, Quality Assurance had prepared and approved an outline or other guidance for scheduling conducting or otherwise controlling i

the training program.

B.

The customer's contract documents specify the ASME code class of the items purchased.

ASME Section III, Division 1, Subsection NB and NC 4367 required the test assemblies for Manual, Machine, and Semi Automatic Welding, includ-ing hardfacing, to be sectioned, and each section examined at 10x to ISx magnification. The code also address the blending of weld repairs into the base material, and verifying that weld surfaces are suit-able for performing Liquid Penetrent or Magnetic Particle examinations, which are accomplished by visual inspections.

Contrary to the above, there was no evidence that a procedure had been developed to control visual inspections, by specifying inspection methods, aids, illumination, techniques and acceptance standards.
