Docket No. 50-382 IE Bulletin 79-28 Possible Malfunction of NAMCO EA180 Limit Switches at Elevated Temperatures

Dear Mr. Seyfrit:

Waterford 3 has installed and plans to install NAMCO EA180 limit switches on safay-relatad valves and dampers. Therefore, a program was established to locate, inspect, identify and correct defective limit switches. Letters were sent to suppliers of equipment on which EA180 limit switches might be used advising them to take corrective action, if necessary. All appli: able valve and damper purchase orders and supplements were reviewed in order to ascertain all equipment with EA180 limit switches.

1 Saventy-seven valves were identified as having EA180 limit switches.

Fifty-one valves had been received on site and were inspected. Fifteen limit switches from the defective lot were discovered, tagged and recorded on Discrepancy Notices.

The defective limit switches are associated with the Safety Injection System valves 2SI-L101A, 2SI-L102B, 2SI-L103A and 2SI-L104B, aid the Component Cooling Water System Valve 3CC-F240A. In addition, the inspect.on found four limit switches with no number stamped on the conduit boss. These switches are therefore 4

assumed to be defective. The unidentified limit switches are associated with the Component Cooling Water System Valves 3CC-TM148, and 3CC-TM149.

Thirty-nine dampers were identified as having EA180 limit switches.

Site in-spection confirmed the vendor's statement that the limit switches supplied were not of the lot with questionable gasket material.

Mr. Karl. V. Seyfrit LPL 13926 Page 2 All additional valves and dampers received on site will be inspected for the defective limit switches as part of the receipt inspection. The last valves and dampers with EA180 limit evitches are scheduled to arrive on site by November, 1980.

The nineteen known limit switches with defective or potentially defective top cover gaskets will be replaced by December 1,1980.
