Dear Mr. Grier:

The reference letter forwarded to NRC pursuant to 10CFR50.55(e) regula-tions, a report involving generic deficiencies in electrical penetration assemblies delivered to Callaway and Wolf Creek Sites. The reference letter further indicated that the generic report was being treated as a final submittal under the 10CFR50.55(e) definition, but that the NRC would be informed in the event additional difficulties or complications arise during the follow-up implementation phase.

The purpose of this letter is to advise the NRC that additional inspec-tions at Wolf Creek site have identified further concerns regarding the quality and integrity of penetration wire terminations and that these concerns extend to other wire size connections in addition to the No.

14/16 connectors addressed in the initial report. Specifically, a num-ber of terminations, including terminations reinspected by Bunker Ramo and certified to be satisfactory, have since been found with insulation I extending into or through the terminal lug barrel. Additionally, a No.12 wire module was identified on site with incorrectly sized lugs.  ;

SLNRC 80-30 Page Two The continuing difficulties with the Bunker Ramo items, and, resultant concerns regarding the effectiveness of this supplier's quality program, has resulted in a decision to replace on site all penetration termina-tions other than terminations larger than No. 12 size. These larger sized terminations are relatively few by comparison and will be inspected to determine the quality of the crimped connections and replaced if found to be either defective or questionable. The schedule for these activities is being developed separately at each jobsite and will be implemented in a manner consistent with site needs. Other quality related concerns, includ-ing possible handling mishaps alluded to in Bunker Ramo's May 12, 1980 report to the NRC per Part 21 regulations, will be handled in accordance with existing site nonconformance control procedures. Loose terminal screws will be corrected when the terminal connectors are inspected or replaced.

There is presently no further fabrication of penetration assemblies under-way at Bunker Ramo. A hold on additional fabrication has been established by SNUPPS pending an assessment of this supplier's quality program and follow-up implementation of required strengthening actions. A management assessment of the Bechtel surveillance inspection effort at Bunker Ramo has also been requested and will be followed up as appropriate.

The above information is to be treated as a supplement to the reference report. In the event additional actions are required, other than those indicated above, the NRC will be so advised. In the interim, any ques-tions concerning this matter are to be addressed to the undersigned.
