POTENTIAL FAILURE TO COMPLY OF TEMPERATURE DETECTION UNITS Gentlemen:
CTI-Nuclear was recently informed by the Edison Electronics Division of McGraw Edison of Manchester, New Hampshire (on February 12,1980) that an electronic component similar to devices supplied to CTI-Nuclear failed in service.
The device used by CTI-Nuclear is McGraw Edison P/N 377-02831, and is an in-tegral part of electronic control circuit for detecting high temperatures in the airstream, both upstream and downstream of the vertical deep bed gasket-less carbon filters in various air filtration systems.
The device is also sed as part of a control circuit for high temperature thermal cutout of elec-tric air heaters in the same air filtration system.
These controllers are used on CTI-Nuclear Part Numbers D-31441 and D-31250.
McGraw Edison attributes the only know failure to use of an acid flux in seal-ing some of the controls during manufacture.
McGraw Edison has concluded that there is no convenient method of determining the field if the control was sealed with acid flux.
The acid flux reportedly could cause corrosion in the controller and result in failure.
The mode of a failure is unpredictable, a failure could cause the contcoller to give erroneous indication such as,1) failing to open and not closing at an alarm point, 2) failing cl'osed and giving a false alarm or, 3) at some intermediate step and functioning inaccurately.

Because thereis reasonable doubt as to the integrity of these devices, we feel that the problem requires reporting 'under the Provision of 10 CFR 21.

O CTI-NUCLEAR Mr. Marc Kenyon May 3, 1980 McGraw Edison has issued a recall notice to its customers to return the control-1ers for repair or replace the units.
We request that you remove the electrical panels described in Attachment 1, and return them to:
CTI-Nuclear 1922 East 107th Street Cleveland, Ohio 44106, ATTN: D.L.KAMMERZELL CTI-Nuclear will then remove the Edison Electronics controllers, and forward 3,
them back to the manufacturer for inspection.
JY CTI-Nuclear detennined that this potential problem required reporting to the NRC in accordance with the requirements of 10 CFR 21.
CTI-Nuclear made such notifi-cation to the NRC on April 30, 1980.

Please consider this document your fonnal notification of the potential failure to comply.
CTI-Nuclear will pay reasonable and documented backcharges for the labor, freight, and overhead required to remove and return the panels to us.
