O Tube Turns Division of Chemetron Docket No. 99900267/80-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on February 19-22, 1980, it appears that certain of your activities were not conducted in accordance with NRC requirements.
Criterion V of Appendix B to 10 CFR 50 states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative accep-tance criteria for determining that important activities have been satisfactorily accomplished."
Deviatious from these requirements are as follows:
A.
The QA Manual procedure TT22-206, paragraph 3.6 states, "The Manager of Production is responsible that copies of welding procedures are available at the welding stations."
Contrary to the above, production tack welding was observed being performed on an 18.5" 0.D. carbon steel pipe for production order HPM 49-3251, Item 001, without having the designated welding procedure TT11-1803, Revision 3, at the welding station.
B.
The QA Manual procedure TT22-206, paragraph 5.4.1 states in part, "A Weld Rod Control Record.
. shall be prepared by the Production Foreman showing the approved heat of wire, flux or rod.
Paragraph 5.4.2 states in part, "The welder keeps the copy of the Weld Rod Control Record with the material as a record of the.
wire used and lot of flux used.
Contrary to the above, the Weld Rod Control Records for wire and flux held by the welding operator performing the production welding on a pipe for Order HPM 49-3256, Item 002, were identified as being for Order 48-7845, Item 001.
