Design and Fabrication shall not he conducted under this QA Prograr.
The OA Progran is implemented as shown on tne attached organization chart.
The Radiation Safety Of ficer is responsible for overall adtrinistratior.

Quality Assurance Prooran The management of CHAP.LEST04 NAVAL SHIPYARD establishes anil iriplements this QA Progran.

The QA Progran will ensure that all defined QA procecurcs, engineering procedures, and speci fic provi>*ons of the packaac design approval are satisfied.
The QA Program will emphasire control of the characteristics of the package which are critical to safety.
The Radiation Safety Officer shall assure that all radioactive material shipping packages arc designed and manufactured under a QA Program approved by Huclear Requiatory Corer.iission for all packages designec or fabricated af ter July 1,1978.
This requirement can be satisfied e
by receiving a cortification to this effeet from the manufacturer.
3 Docunient Control

All documents related to a specific shipping package will be controlled through the use of wri tten procedure:s. All document changes will be performeil according to written procedures approved by management.

Shipments will not be made unicss all tests, certifications, acceptances, and fina! laspections have been completed.

Radiography personnct shall perform the critical handlino, storage and shipping operations.
5 Inspection. Test and Operatina Status inspection, test and operating sta.tus of packages for certain specia!
fora radioactive material will be indicated and controlled by written procedures.
Status will be indicated by tag, label, r.orking or 109 entry.
Status of nonconforming parts or packages will be positively maintained by written procedurcs.
Radiography personnel shall perform the regulatory required inspections and tpsts in accordance with written procedures. The Radiation Safety Officer shall ensure that these functions are performed.
6.
Quality Assurance Records Records of package approvals (including references and drawings),
procurement, inspections, tests, operating logs, audit results, personnel training and quali fications and records of shipments will be 'r.aintained.
Descriptions of equipment and aritten proccoures will also be maintained.
These records will be maintained in accordance with written procedurcs.
The records will be identifiable and retrievable.. A list cf these records, with their storage locations, will be maintained by the Radiation Safety Officer 7.
Audits Established schedules of audits of the QA Pronram will be performed using written check lists.
Results of audits will be maintained and reported to managernent. Audit reports will be evaluated and deficient areas corrected.
