Areas Insoected:
Follow-up on Construction Deficiency Reports dated December 11, 1979, and January 2, 1980, generated by Duke Power Company relative to wire insulation not being stripped sufficiently, thus allowing the insulation to be compressed in the connecting lugs. The inspection involved 10 inspector-hours on site.
Results:
In the one area inspected, no deviations or unresolved items were identified.

Follow-up on Construction Deficiency Reports:

Introduction Construction Deficiency Report notifications were generated by Duke Power Company (DPC) on December 11, 1979, and January 2, 1980, relative to McGuire Units 1 and 2, and Catawba Units 1 and 2, respec-tively.
Both reports dealt with wire insulation on cables to motor control centers (MCC) not being stripped sufficiently, resulting in insulation being compressed in the connecting lugs. The inadequate connections have a potential of overheating and/or grounding.
2.
Objectives The objectives of this inspection were to:
ascertain whether or not there were generic implications; what corrective action has been taken, and what action has been taken to preclude recurrence.
3.
Method of Accomplishment The preceding objectives were accomplished by:
a.
Review of Construction Deficiency Reports.
b.
Review of DPC procurement documents to Nelson Electric Company, including purchase orders, equipment specifications and draw-ings.
c.
Review work instructions, test procedures, inspection check-lists, and Certificates of Inspection.
d.
Observation of Wire Pull tests and review calibration records of Crimp Tools.
3 Observation of inspections being performed on MCCs.
e.
f.
Discussions with cognizant personnel.
4.
Findings DPC placed orders with Nelson Electric Company (NEC) for 117 MCCs for the McGuire Units and 149 MCCs for the Catawba Units.
All MCCs for McGuire have shipped and there is a balance of twenty (20) remain-ing to be shipped to Catawba.
It should be noted that DPC is the only licensee using the safety related NEC MCC.
Prior to discovery of the problem, there was no specified operation in inspection procedures or on inspection checklists, to verify that insulation 'Aripping had been accomplished so as to prevent compressing insuia; ion in the connecting lugs.
Each MCC is source inspected by DPC and a Certificate of Inspection is generated and signed by both NEC and DPC.
NEC, when notified of the problem, sent a quality control inspector to the DPC McGuire site.
He, in conjunction with DPC quality repre-sentatives, inspected seven MCCs between February 11-14, 1980.
Each MCC has seven (7) to twelve (12) pans (depending upon function) with as many as nine (9) connections in each, which could be involved in the problem area. The inspection revealed two (2) or three (3) loose connections. There were a couple of instances in which the connections were questionable regarding the insulation being compressed.
As a result, both DPC and NEC made revisions to their inspection procedures.
DPC procedure titled " Equipment Inspection Trip Report" was revised (revision 3), on January 30, 1980.
This procedure, used by the DPC source inspector, incorporated item D.14 under the heading of Inspection which states, " Wiring - Cable properly stripped and only bare conductor inside lug."
NEC revised procedure number J101.035.2 " Inspection Procedure" to revision 4 on January 9,1980, to include, " stripping of wire and cable insulation, and insertion into connector lugs, shall insure full contact of the base conductor to the lug."
In addition, procedure J020.48, " Inspection Check List For Class 1035 Motor Control Centers" was revised on February 10, 1980, to revision 2, and included Item 5 which states, " Verify wiring pro-perly stripped and only bare conductor inside lug."
This was identified as being critical, which requires particular attention
4 during inspection.
Observations by the NRC inspector during NEC's final inspection showed that this was being accomplished.
It should also be noted, that DPC in their telephone notification to NRC Region II, indicated that all NEC MCCs would be inspected for this condition.
It appears that the actions taken, will correct the reported defi-ciencies and prevent their recurrence.
C.
Exit Interview An exit meeting was conducted at the conclusion of this inspection on Februa ry 27, 1980, with the following management representatives in attendance:
