~ NOTICE OF VIOLATION Rensselaer Polytechnic Institute Docket No. 50-225 Troy, New York 12181 License No. CX-22 Based on the results of an NRC inspection conducted on January 15-17,1980, it appears that one of your activities was not conducted in full compliance with the conditions of your NRC license.
This is a deficiency.
10 CFR 20.401 requires each licensee to maintain records showing radiation exposures of all individuals for whom personnel monitoring is required under Par 20.202 of the regulations in this part.
Such records shall be kept on Form NRC-5, in accordance with the instructions contained in that form or on clear and legible records containing all the infomation re-quired by Form NRC-5. The doses entered on the forms shall be for periods of time not exceeding one calendar quarter. Form NRC-5 requires that the doses be recorded in units of rem.
Contrary to the above, as of the dates of this inspection, the personnel monitoring records for 1978 and 1979 maintained by the licensee were defi-cient in that, records for some individuals contained no exposure information; exposure information was in units other than rem; and/or the records of exposure were for time intervals which were undefined or for intervals exceeding one calendar quarter.
