RHSB INFORMATION REPORT * #7: UPDATE ON EPIDEMIOLOGIC STUDIES OF PORTSMOUTH NAVAL SHIPYARD WORKERS I

The panel of consultants overseeing the NIOSH study of Portsmouth Naval Shipyard (PNS) workers met on March 7,1980 to review the progress of the study and to receive preliminary results of Dr. T. Colton's reanalysis of his Proportionate Mortality Study. Dr. Colton's work used employment history data, obtained by NIOSH for its investigation, to verify the findings of the Najarian/Colton study reported in Lancet (June 13, 1978).

We reported previously on a presentations ef these ne+ data by Dr. Colton (RHSB Information Report #3). Since that time, Dr. Colton has refined his analysis by adding non-nuclear workers, nosologist-coded causes-of-death, and number of years that workers wore badges. Mortality for 697 non-nuclear and 365 nuclear workars was compared with that for the U.S.

white male population.

Both non-nuclear and nuclear workers demonstrated greater than expected mortality from respiratory cancer, due apparently to factors other than radiation exposure. A dose-response relationship j

was demonstrated for a category called " hematologic cancers and diseases I

of the blood", although the number of deaths in this category was very small and the category included all leukemias and lymphomas. The final report of this study is due in April.

NIOSH is conducting a retrospective follow-up study of some 27,000 PNS workers along with an industrial hygiene study of the shipyard.

Presently, NIOSH has successfully followed-up (i.e., determined the vital status) of over 80% of the original study cohort. By August, when the study is to end, at least a 90% follow-up rate is expected. As there seem to be other potentially hazardous exposures among PNS workers, in addition to radiation, NIOSH expects to investigate further any excess disease-specific mortality found in the follow-up study through a subsequent case-control study. The case-control approach, which focuses on a particular cause of death, allows l

in-depth examination of the risk for specific jobs and for other etiologic factors such as cigarette smoking.
