Docket No. 99900025/79-03 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on December 5,19 and 20, 1979, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:

Criterion V of Appendix B to 10 CFR 50 states:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawicgs, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished." Deviations from these requirements are as follows:

Paragraph 18.07 in Part Two of Ebasco Specification No. CAR-SH-M-30 states, " Materials used for safety-related systems which are designed for pressure in excess of 275 ; wig or temperature in excess of 200 F, shall meet the testing requirements of Sections NB 2552, NB 2560 (Applicable Code summer 1973 addenda) except for seamless pipe, tubes and fittings 2 in.

nominal pipe size and smaller as per ASME III Subsection NB 2510.

Applicable to Clas; i, 2 and 3 piping with the above design conditions and as follows:

All forged and/or extruded pipe and fitting shall be ultrasonicalgy examined in two directions circumferentially using angle beam (40 to 75 ) techniques referenced to a 5 percent T (T= Wall thickness) notch.

Penetrant tests or magnetic particle tests shall be performed on the OD surfaces of all pipe bends having a radius of curvature less than twenty (20) times the nominal pipe diameter and on the OD and accessible ID of all Safety Class 1 fittings.

For those material conditions and product configurations (i.e., small fittings, tees, etc.) in which examination method is ineffectice, the alternative methods of Section NB 2551 shall be used."

Contrary to the above:

Applicable ASME Section III Code, Sub-Section NB-2551, 2552, 2560 material nondestructive examinations were not specified by the Southwest Fabricating and Welding Co. (SWF) Engineering Department nor were these examinations performed on all items in eight (8) piping spools, which were required to be furnished in accordance with Specification No. CAR-SH-M-30 for installation in ASME Section III Class 3 safety related piping systems with design pressures and/or temperatures in excess of 275 psig and 200 F, respectively.
