b Armco Incorporated Advanced Material Division Docket No. 99900268/80-01 NOTICE OF DEVIATIONS Based on the results of an NRC inspection conducted on January 14-18, 1980, it appears that certain of your activities were not conducted in accordance with NRC requirements.
Criterion V of Appendix B to 10 CFR 50 states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, pro-cedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
Deviations from these requirements are as follows:
A.
With regard to internal audits, the QA Manual paragraphs, as follows, state in part, "16.2.5.1 Corrective action on deficient areas will be initiated within seven days of the filing of the Auditor's report."
"16.2.5.2 Within thirty days the QA supervisor will send a report to Management defining the deficient areas, identifying the problem causing the deficiency, and reporting the nature of the corrective action taken to prevent reoccurrence."
"16.2.5.3 Within sixty days the QA Supervisor will reaudit those areas reported deficient.
Contrary to the above, the above required actions were not performed to resolve the deficiencies identified in the two (2) March, 1979 internal audit reports in the areas of procurement control, process control, heat treatment and hydrostatic testing, and calibration.
B.
With regard to AMD and a subcontractor's nondestructive examination (NDE) personnel examination and certification, the QA Manual, paragraphs as follows, states in part, "9.3.2.1 They will qualify in accordance with SNT-TC-1A, " Supplements and Appendicies," 1975 Edition...
"9.3.4 NDE may be performed by an outside testing laboratory provided:
9.3.4.3 Training and qualification records of operators.
shall be kept in A.M.D files.

2 The Recommended Practice No. SNT-TC-1A, June 1975 Edition, paragraph 8 Examinations, requires that an eye examination shall be administered to NDE personnel on an annual basis and the results shall be kept on file for the period of certification.
Contrary to the above, there were no eye examination records dated later than August 8, 1978 on file at A.M.D for a subcontractor's four (4) ultrasonic test technicians and for one (1) A.M.D NDE technician.
C.
With regard to postweld heat treatment furnace temperature charts, the QA Manual paragraph 7.7.3 states in part, " Charts are identified with the Mill Order number, piece number, and charge number."
Contrary to the above, the Mill Order number and piece number were not identified on the furnace chart records for postweld heat treatment of piping for Mill Orders No. HMS 9-7668, Piece IP and No. HMS 9-7356, Pieces IP, 2P and 3P, and other Mill Orders.
