O Colt Industries Fairbanks Morse Engine Division Docket No. 99900300/80-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on January 21-25, 1980, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR 50 states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished." Deviations from these requirements are as follows:
A.
Paragraph 8.0 of Manufacturing Engineering Standard No. 00:01:02, dated Februa ry 10,1978 (page 2 is dated February 27, 1978), states, "Manu-facturing Engineering will maintain and issue a list of the Manufacturing Engineering Standard revision and addendas applicable to contract sales order number.
This list will be issued to all Manufacturing Engineering Standard manual holders."
Contrary to the above, a list of Manufacturing Engineering Standards (MES) revisions and addenda applicable to contract sales order number had not been maintained and issued by Manufacturing Engineering to all MES manual holders.
B.
Paragraph 5.4.1.2 of Section 5, Revision 2, dated January 1978, of the Quality Assurance Manual, states in part, "The Process Operations Sheet informs the shop how to manufacture the part by identifying the type of operation, the normal sequence of the operations, the machine tools to be used and the tools required to perform the operations. The Process Operation Sheet shall also identify the in process and final inspection operations to be performed on the product.
Each operation, when completed by the operator, shall be initialled and dated by either the operator or his supervisor."
Operation No. 0140 of Operation Sheet, Data Process (D/P) date of.May 24, 1979, Part No. P12602841, Serial No. 9060, Order No. 418740, states in part, " Note: All M68-3SMA His to be checked with gage by operator after each individual hole is completed.
Operator is required to sign off and state condition of Main B-- stud holes in the following spaces provided on the Process Document f.
each main brg. Main Brg #1, Main Brg #2...
Main Brg #9."

2 Contrary to the above, the operator had not signed off and stated the condition of main bearing stud holes for main bearing numbers 1 and 2, which had been completed.
C.
Paragraph 10.4.4.2 of Section 10, Revision 0, dated April 1976, of the Quality Assurance Manual, states in part, "Unless modified by a Quality Control Inspection Instruction.
. the inspection operations will be as defined on the Process Document.
In those instances where inspection operations need not be performed at this point in the cycle or where operations are run out of sequence, the inspection operations may be post-poned until later in the Manufacturing cycle. This condition shall be so noted on the document by the inspector, in the form of a statement that lists the specific postponed operations performed as a part of a subsequent inspection operation."
Contrary to the above:

The inspector had not placed a statement on the Operation Sheet for an apparent postponed operation (0015), which required inspection but exhibited no inspection activity.
Subsequent inspection operations had been completed.
This condition was noted on Operation Sheet, D/P date of December 14,1978, Part No. P12609673, Order No. 418720.
2.
The inspector had not placed a statement on the Operation Sheet for an apparent postponed operation (0050-required recording of serial number, actual dimension and parallelism of sides (which had been lined out without benefit of signature or date)). Subsequent inspec-tion operations had been completed. This condition was noted on Operation Sheet, D/P date of March 3,1979, Part No. P12609673, Order No. 418720.
