Docket No. :50-334,1 License No. DPR-66 ) - Report of Facility Changes, Tests and Experiments-The following is a compilation of facility changes, tests and experiments completed during'1979 which did not require prior NRC approval and are being reported.in accordance with 10-CFR 50.59. ' Design Change DC0097 'I l -Design Change DC0097 entitled " Increase Capacity of. Spent Fuel Pool with i High Density Racks".was completed. /D11s change increased the number of spent fuel racks to 17.an'd the number of cells to 833. The new racks are equipped with more cells with no spacing between individual cells. Additionally, new' seismic. bracing, two (2) failed fuel assembly containers and lighting modification were also installed. The safety evaluation concluded that the change did constitute an -{ unreviewed safety question since the probability of occurence or consequence of an accident had been increased due to the additional decay heat in the pool. As a result;of this question, the BVPS #1 Technical Specifications i were modified to include a requirement, that in order to unload a full core (with-12 regions previously stored in the pool - most limiting condition).. a 135 hour interim ~after reactor shutdown must elapse as a means of limiting the pool' water bulk temperature to 1840F maxi =um. The 135 hour delay time will provide adequate short lived' fission product ~ decay and associated thermal effects.^ l ' Design Change DC0115 Design Change DC0115. entitled " Add Tritium Sampling in Gaseous, Waste" was completed. This change added the capability to sample gaseous waste hold-up tank gas for tritium presence prior to release to the atmosphere. g .The actual change-involved installation of stainless steel tubing as well as isolation and pressure regulating valves in the inlet line of the radiation monitor in the gaseous waste system. --The. safety. evaluation concluded that the change did not constitute an unreviewed safety question. The basis for-the evaluation was'that the change met the requirements of the' original design and that the probability of an ~ accident _or equipment malfunction is not increased. Also, no reduction in safety margin has 'been experienced by -the subject change. Design Change DC0133' Design' Change'DC0133' entitled'" Add Pressure Reducers on Steam Generator Sample Lines" was: completed. This change involved the addition of. pressure reducing regulators on cach of the three blowdown sample lines upstream of .the: sample sink. The pressure. experienced by the: ion' exchanger upstream-of. the sampleisink was excessive :with resin being discharged from the exchange .30040102FO .m . =

The pressure reducing regulators solved both of these problems. This change'did not-constitute an unreviewed safety question with the original design criteria remaining unchanged. Design Change DC0137 Design Change DC0137 entitled." Reactor Coolant. System.0verpressure Protection" was completed. In this. design change, the Power Operated Relief 4 Valves were. modified to provide a dual set-point such that over-pressure protection would be available when the plant was-in cold shut-down. In 1 conjunction with this, an additional Nitrogen supply system was added as a back-up to the containment air compressor system. The safety evaluation concluded that the change did not constitute an unreviewed safety question. Design Change DC0218 Design Change DC0218 entitled " Component Cooling Water Change for Air Compressors" was. completed. This change installed hard piping from the station's filtered water system to'the three (3) air compressors. Originally, when CCT system was not in service, cooling to the compressors was provided, using. filtered water connected by temporary rubber hoses. The compressors now have redundant cooling depending on the mode of the station.- 1 This change did not constitute an unreviewed safety question with the original design criteria remaining unchanged. Design Change D'CO220 Design Change DC0220 entitled " Replace Main Transformer with Temporary Westinghouse Transfor=er" was completed. :In this design change, the installed General' Electric main transformer was damaged and temporarily replaced by.a Westinghouse manufactured unit. A primary to secondary direct fault caused a short in the circuitry and resulted in an explosion with considerable internal ' damage to the installed transformer. While the unit was in' repair, a -Westinghouse transformer was substitu'ted. .It should be noted that the General l Electric unit is currently being re-installed. 'This change didinot constitute an unreviewed' safety question with the . original design criteria remaining. unchanged. Design Change'DCO224 Design Change DCO224 entitled " Add Chemical Feed Tank for Auxiliary -Boilers" was completed. This design: change involved modifying the chemical ~ W W .~.

The actual change involved installation of a new chemical addition tank, piping from the chemical addition pump to the tank and cross-over piping on the inlet and discharge of the two (2) chemical addition tanks. This changt did not constitute an unreviewed safety question as the change met the requirements of the original design. Design Change DC02 {5 Design Change L'C0225 entitled " Generator Gas Duct RTD* Installation" was completed. fin tt'is design change, Westinghouse requested that additional temperature monitors be installed on the Main Unic Generator Stator. Eighteen (IS) additional RID's were installed as a result of this change. To accomplish this, cable and conduit had to be installed to wire the RTD readouts to the station ecmputer. This change did not constitute an unreviewed safety question as the change not only met but exceeded the requirements of the original design. Design Chans;e DCO231 Design Change DC0231 entitled " Design and Erect a Decon Room in the Primary, Auxiliary Building" was completed. This design change installed a Decontamination Room in the Primary Auxiliary Building on the 768 level. This room is complete with hot and cold water, air filtration system, lighting, sink, drains, tables and shelving. In addition, a concrete curb has been installed around the room to prevent any seepage of fluids outside the rooc. This change did not constitute an unreviewed safety question since no change to any safety system or structure had occurred. Design Change-DCO251 Design Change DC0251 entitled " Deletion of Low Pressurizer Level Coincident with Low Pressurizer Pressure in the Solid State Protection System".was completed. This change modified the Protection System logic from coincident low pressurizer pressure and level to 2 of 3 low pressurizer pressure for safety. injection. The basis for this change was to prevent a s=all steam break on top of the pressurizer from not ' initiating safety injection. The safety evaluation concluded that the change did not constitute an unreviewed safety question ani that the change.uas developed as a response

Design Change DC0255 Design Change DC0255 entitled " Slot Main Coolant Pump Structural Beams to Allow for Pump Thermal Movement" was completed. Installation of a new
- main coolant pump hydraulic lifting system resulted in slotting two existing structural ^ beams to allow for pump thermal movement.
The new main coolant Pump stands were slightly larger than the existing stands. Slotted benms were repaired to provide additional strength members to compensate for the slotting. The safety evaluation concluded that the change did not constitute an unreviewed safety question. In fact, the safety of the plant was improved by the modification since it will allow unobstructed thermal expansion of the reactor coolant pumps and piping during plant heat-up. Design Change DC0259 Design Change DC0259 entitled " General Electtp_ Modifications to Inside Recirculation Spray Pump Motors" was completed. As a result of a seizing problem at another station with the original bearing design, General Electric initiated a change notice to remove and replace the lower thrust bearing on the Inside Recirculation Spray Pump Motors. Installation of the new bearing resulted in minor machining of the bearing mating surface and installing a new design lower slinger. The safety evaluation concluded that the change did not constitute an unreviewed safety question as the safety of the motor has not been compromised, nor has the plant safety posture decreased. Design Change DC0263 Design Change DC0263 entitled " Change-Out 4160-480 Volt Transformers (Emergency)" was completed. This change qualified and installed four (4) Unic No. 2 (ITE) 1000 KVA/1333KVA 4160 - 480 Volt normal bus transformers in Unit No. 1. These transformers were used as replacements for the original-Unit No. I emergency bus transformers (GE) 8N, 8N1, 9P and 9Pl. The change was completed by installation of the four Unit No. 2 transformers. In this change, the original General Electric transformers were replaced
- with'four (4) ITE transformers.
The safety evaluation concluded that the change did not constitute an unreviewed safety question. .The' evaluation noted that the ITE transformers .are wound Y-Delta whereas the original GE transformers are wound Delta-Delta.
- Since the replacement transformers have.a higher insulation level and are built to the-same or better specifications than the original transformers, i.

Tests and Experiments -(continued) the plant safety posture is better than with the GE transformers and equal to that assumed in the SAR. Design Change DC0269 Design Change DC0269 entitled " Modification of Suoport PS-2 or 3" WR-56 was completed. This change involved modificatica of the support to provide maintenance easy access when working on the "A control room ~ air-conditioning compressor. The new support is designed such that it will allow removal.of the compressor without removing the support. The safety. evaluation concluded that the change did not result in any unreviewed safety questions. It was determined that the new support is as functionally adequate as the original support and will enhance the maintainability of the freon compressor. Design Change DC0270 Design ~ Change DC0270 entitled " Replace 1A Charging Pump with Unit No. 2 Model (CH-P-1A)" was completed. This design change replaced the 1A charging pump with a BVPS Unit No. 2 model. The change-out was essentially a one tor one replacement with the exception of the additional brace on the balancing line of the Unit No. 2 pump and elimination of river water to the seals as { the Unit No. 2 pump has a cross connect system. The safety evaluation concluded that the change did not constitute an unreviewed safety-question since the Unit No. 2 pump satisfies the original pump specifications and performs its intended function. Design' Change-DC0273
- Design Change DC0273 entitled " Boric Acid Purification Line Piping" was completed.
The boric acid system was modified at the request of the NSSS supplier in order to establish a method for cleaning the system. A new line was installed with the capability of recirculating the boric acid system through the Deborating Demineralizers (CH-I-3A & 3B). The intent was to remove soluable chlorides from the system thereby reducing impurities in the. boric acid and reactor coolant systems while maintaining system integrity and. reducing personne1' radiation exposure. - The safety evaluation concluded that the change' did not constitute an unreviewed safety question and that the design change does not alter the basis as described in the.SAR. Desien-Change DC0274 Design Change DC0274 entitled:"Use Unit #2 Coupling on Unit #1 Charging Pump CH-P-16" was completed. This change resulted in' removal of the motor / -

The original style coupling as supplied on the IC Charging Pump is no longer available. The safety evaluation concluded that the change did not constitute an unreviewed safety question since the replacement coupling is currently supplied with the Unit #2 Charging Pumps and the new coupling will perform the same function as the original. i FSAR, Table A.2.2-1 FSAR, Tabis A.2.2-1 entitled " Matrix of Appendix B, 10CFR 50 vs. Operations Quality Assurance Program" was altered. The operation program numbers were incorrectly submitted in the original Safety Analysis Report.' The attached Table reflects the correct numbers and titles of the Operations Quality Assurance Program. This change did not constitute an unreviewed safety question as the misnumbering of the Procedures did not compromise the Q. A. Program at BVP5 #1. Regardless of the number of the Procedure, n m intent of the ~ station is to maintain strict compliance to the enti c. crogram. l Station Administrative Reorganization The Station organization as described in Section 12 of the FSAR has been revised. The revised organization has been previously submitted to i NRC as Technical Specification Change Request No. 45. ) i . FSAR Appendix A Page A.2-15 The frequency of the-Management Audit of the Duquesne Light Company Q.A.-Program was changed from 12 to 24 months by the Offsite Review Committee. The-ORC. determined that no unreviewed safety question was involved since 1 - the scope of the audit was unchanged and it was used by management to ensure adequate implementation of the Q.A. Program.
