O Appendix A NOTICE OF VI0I.ATION Union Electric Company Docket No. 50-483 Based on the results of an NRC inspection on February 27-29, 1980, it appears that certain of your activities were not conducted in compli-ance with NRC requirements as noted below. This item is considered to be an infraction.

10 CFR 50, Appendix B, Criterion V requires, in part, "that activities affecting quality shall be prescribed by documented instructions... and shall be accomplished in accordance with these instructions."

a)

Paragraph 10.1 of Specification 10466-C103(Q) states, " Vibrators shall not be use.d to move or spread concrete."

Paragraph 7.1 of ACI 309 states, "The layers should be as level as possible so that the vibrator does not need to move the concrete laterally, since this might cause segregation."

Contrary to the above, the inspectors observed on the lower lift of concrete pour No. UC306W35 on the ESWS Pump House on February 28, 1980, in the center of the west wall, that a vibrator was left in the concrete for two to three minutes to facilitate concrete move-ment under two blockouts in the wall.

Paragraph 4.2.4 of Daniel Construction Procedure QCP-106, Revision 8 states, " Verify Reinforcing Steel is securely tied to prevent displacement during construction and concrete placement Paragraph 7.7 of Specification 10466-0112(Q) states, "Special care shall be exercised to prevent any disturbance of the rein-forcing bars in concrete that has been recently been placed."

Contrary to the above, the inspectors observed that vertical re-inforcing bars were untied and moved to facilitate concrete place-ment for the west wall corners of pour No. UC306W35 in the ESWS Pump House on February 28, 1980. The rebar was replaced and retied when the pour reached the required elevation.
