Lakeside Bridge & Steel Company Docket No. 99900342/79-02 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on December 10-14, 1979, it appeared that certain of your activities were not conducted in full com-pliance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR states,
" Activities affecting quality shall be prescribed by documented in-structions, procedures, or drawings, or a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative accept-ance criteria for determining that important activities have been satisfactorily accomplished."
Deviations f rom these requirements are as follows:
A.
Welding Procedure Specification (WPS) 30-B-U3-GF revision 1, which was observed being used on Order 9182 on two different occasions, required an interpass temperature range of 70 F to 500 F to be main-tained during welding.
Contrary to the above, verification that the interpass temperature range of 700F to 500oF was being maintained during welding was not being performed, in that the two welders involved in depositing the welds were not equipped to verify the interpass temperature require-ment.

QA Manual Section 8, paragraph 8.1, states in part, " Purchase of ma-terials, parts, fabrication, heat treating and examination or testing services is from those vendors who have been approved by Quality Assurance and whose names appear on the current Approved Vendor List (AVL)
. based upon Vendor Evaluation.
Vendor Evaluation and/or vendor audits are performed.
. and are based upon written checklists.
., pertinent to the activity the vendor performs, such j
as material manufacturing... processing and testing, NDE, or fab-rication."
i Contrary to the above, two vendors have been placed on the AVL in which the performed evaluation was not based upon written checklists pertinent to activities performed _by the vendors.
