Description of Event

On February 27, 1980, during normal operation at 100% power, it was discovered that the "B" gaseous vaste filter charcoal adsorber was operated for greater than 720 hours0.00833 days <br />0.2 hours <br />0.00119 weeks <br />2.7396e-4 months <br /> (934 hours0.0108 days <br />0.259 hours <br />0.00154 weeks <br />3.55387e-4 months <br /> total) without perfonaing a laboratory analysis of a carbon sample as required by T.S. 4.6.4.3.C.

Probabic Consequence of Occurrence

--- The consequences of this event were-limited..because the redundant. "A"..

gaseous waste filter was placed into operation as soon as it was discovered that the surveillance interval of the "B" filter had been exceeded.

As a result, the health and safety of the general public were not af fected.

There are no generic implications associated with this occurrence.

Cause of Event

The time limit was passed because the charcoal filter timers, which integrate the amount of time the filters operate, were only recently installed by a design change and the 720 hour0.00833 days <br />0.2 hours <br />0.00119 weeks <br />2.7396e-4 months <br /> surveillance requirement for the carbon sample analysis was never included in the operators' logs.

Immediate Corrective Action

The "B" gaseous waster filter was isolated and the "A" filter was put into service.

The charcoal in the "B" filter was then replaced and a fecon leak test to check adsorber ef ficiency was performed satisfactorily.

Scheduled Corrective Action No corrective action is required.

Actions Taken to Prevent Recurrence Operators' logs and applicable periodic test procedures have been revised to include the 720 hour0.00833 days <br />0.2 hours <br />0.00119 weeks <br />2.7396e-4 months <br /> surveillance requirement for the charcoal filters.
