ATTACHMENT.A Response to Notice of Violation Notice of Violation:
A shipment of licensed radioactive taterial was made by the licensee to Barnwell, South Carolina on December 14, 1979, in an exclusive use vehicle, in which the dosa rate as measured from the licensed material was 15-17 millirem per hour at 2 meters (six feet) from the vertical planes projected by the outer lateral spr-faces of the vehicle, upon receipt of the material at Barnwell, South Carolina on December 17, 1979.
This is a Violation.
(Civil Penalty $5,000)
Response; Reasons for Items of Noncompliance (A) ' Lead blanket shielding improperly rigged.
(B) Normal fluctuation in radiation survey instrumentation due to + X % in calibration of instruments and the sensitivities of different types of instruments.
Corrective Action:
(A) Re-instruction of all Health Physics Supervisors, Waste Management Supervisors and Technicians on the proper methods of taking good radiation surveys and the importance,of good packaging and shielding practives.
(B)
Instituted changes in PNPS cask handling and Waste shipment procedure.
(C)
Instituted station policy that only the Chief Radiological Engineer or Sr.
Radiological Staff Engineer have the authority for the final approval to release radioactive waste shipment from Pilgrim Station.
(D) Minimize the practice of hanging lead blankets from a container as a method of shielding. Consider more positive methods of attaching required shielding.
As a result of corrective steps taken, Pilgrim Station was in full compliance as of January 8, 1980.
